    Application Requirements

    1. MAT-1 Maternity Notification Form. This is available from any SSS branch or from their website. Alternatively, you may google and search “sss mat 1 form.”

    2. Proof of Pregnancy – This can be in a form of ultrasound report. If you are employed, ask your HR or your company’s officer in charge on the type of proof they prefer. Other HR requires an original copy of ultrasound (US) report, others a photocopy of US, still others a note from the OB Gyne stating that the applicant is pregnant.

    Application Process
    1. Submit your filled out MAT-1 and proof of pregnancy to your company’s HR.
    2. Your HR will then forward your requirements to SSS. Your MAT-1 needs to be submitted within 20 days of your conception. Failure to do so will cause your application to be denied.

    Things you may want to know:

    Your company may submit your application over the counter at any branch. SSS will stamp your MAT-1 and return it to them. It will remain with them as they need to submit it again after your childbirth.

    Other companies submit to SSS via online through their website. SSS will then send a confirmation email that they have received your application. In both application processes — offline and online — your HR can notify you that your application has been received by SSS.

    AFTER GIVING BIRTH

    Obtainment Requirements
    1. MAT-1 with STAMP from SSS
    2. MAT-2 Maternity Reimbursement Form. This is available in any SSS branch or from their website. Alternatively, you may google and search “sss mat-2 form.”
    3. OB History — you can obtain this from your OB Gyne
    4. Proof that you delivered via NSD or CS. The proof of CS delivery is called CTC of Record of Operating Room. Both forms can be obtained from your hospital’s records section.
    Things you may want to know:

    Other employers give the maternity benefits in advanced to the employee/applicant. They will then reimburse the payment at SSS after your childbirth. Just confirm with your HR if this procedure applies to your company as well.
    Take note that the amount of advanced maternity benefits that your company will provide will vary depending on your type of delivery. CS deliveries have higher maternity benefits than NSD. In your MAT-1, indicate the type of your delivery. If you’re CS, you need to include/attach a note of scheduled CS from your OB Gyne.

    BEFORE GIVING BIRTH

    Application Process

    Submit your filled out MAT-1 and proof of pregnancy to any SSS branch near you. The SSS will stamp your MAT-1 and return it to you. You need to keep the STAMPED MAT-1 as you will submit it again after your childbirth to get your maternity benefits.

    Alternatively, you may submit your requirements online at their website. Be sure to make your SSS online account by registering at their site. This is where you will submit your requirements. The SSS should confirm that they have received your requirements.
